Numerous missing ceiling panels along the back hallway of the facility. [CA] Ceilings of all rooms where nonprepackaged food is handled shall be smooth, of durable construction, and of nonabsorbent material that is easily cleanable. Observed large hole on wall with numerous electrical conduits protruding out. [CA] Repair holes in structure and maintain in good repair to prevent pest harborage.

Ice scoop maintained with handle in direct contact with consumable ice. [CA] Store scoop handle in such a way that the handle does not come in direct contact with food to prevent contamination. Observed ice cream cartons and gallon milk rugs reused to prepare ice and store juiced beverages. [CA] Single use food container (e.g. soy sauce container, milk carton, apple juice container, can food, wine bottle) shall not be reused to store food. Observed two-compartment ware wash sink used as food preparation counter to store sugarcane. [CA] Utilize sinks for their designated purpose. Food preparation shall be conducted at the food preparation sink.
