Observed 3 packages of smoked fish with only name of product on packaging. Fish is measured at 55F; per PIC, fish was brought in today about 1 hour ago. PIC is unable to provide invoices for fish. [CA] Food must be from an approved source or properly documented. Discussed with PIC that fish from unapproved source shall not be sold in facility. [SA] PIC removed 3 packages from sale and will take home.

1. Observed numerous fruit flies near the 2 compartment sink. [CA] Food facility shall be kept free of non-disease carrying insects, weevils, ants, gnats, and fruit flies. REPEAT VIOLATION 2. About 4 dry rodent droppings observed under the produce stand; per PIC, they self treat facility for vermin. [CA] Remove droppings and clean and sanitize all impacted areas. At the re-inspection no live vermin activity may exist of facility will be ordered closed for imminent health risk.
