Food contact surfaces clean, sanitized
Build up of mold like growth inside the ice chute of the soda machine on the sales floor. [CA] Clean, sanitize, and maintain.
Dishes, equipment, or surfaces that touch food were not properly washed, rinsed, and sanitized, or sanitizer was not at the right strength. Clean surfaces help stop germs from spreading to food.

Commercially processed hot dogs and taquitos on the rolling warming unit measured between 118F to 135F. Employee stated that items were placed on the warming unit to be reheated to above 135F approximately 2 hours prior. [CA] RTE commercially processed food shall be reheated to >135°F for hot holding. [COS] Items were VC&D due to improper reheating.
Food was not kept cold enough, hot enough, cooled fast enough, or cooked or reheated to the right temperature. Bacteria grow quickly in the "danger zone" between 41°F and 135°F, which makes this one of the most important food-safety checks.

Dead rodent observed on snap trap below the storage shelf for beverages next to the CO2 cannister by the back door. Employee stated that pest control services facility monthly. Latest pest control report unavailable for review. [CA] The premises of each food facility shall be kept free of vermin. A food facility shall not operate when there is a vermin infestation that has resulted in the contamination of food contact surfaces, food packaging, utensils, food equipment, or adulteration of food(s). The food facility shall cease operation of the food facility immediately. The food facility shall remain closed until: there is no longer evidence of a vermin infestation; all contaminated surfaces have been cleaned and sanitized; and contributing factors such as cleaning, repairs, and the elimination of harborages have been resolved. [COS] Rodent and trap was removed.
The inspector found mice, rats, cockroaches, flies, or evidence of them (like droppings) inside the business, or conditions that could attract them. Pests can spread bacteria to food and surfaces, so this is treated as one of the more serious findings.

Health placard has been removed. Employee stated that the window where the placard was placed was broken and that the placard was discarded. [CA] The placard shall be posted and clearly visible to the general public and patrons entering the food establishment. Notify this Division for a replacement placard when necessary.
A required permit, food safety certificate, trained manager, or posted sign was missing or not current. These show the business is following the rules and has trained staff.

Hand wash sink in the back area by the three-compartment sink lacked paper towels. [CA] Handwashing facilities shall be equipped with handwashing cleanser and single-use sanitary towels in dispensers. Hand soap and paper towels shall be readily available and easily accessible at all times. [COS] Paper towels were provided.
Staff were not washing their hands when required, or a handwashing sink was blocked, missing soap or paper towels, or used for something else. Hands are one of the most common ways germs reach food.

Ventilation fan inside the restroom appears inoperable. [CA] Restrooms shall have an openable screened window, an air shaft, or a light switch-activated exhaust fan, consistent with the requirements of local building codes.
Floors, walls, ceilings, lighting, ventilation, or garbage areas needed repair or cleaning. These are usually less urgent than food-handling problems but still need to be fixed.
