1. Observed Vermin: Rodent droppings in various food and non-food areas including interior of back warehouse storage area, floral section, underneath refrigerator units in produce, under aisle shelves and on shelves in grocery aisle 12, 13, 17, and 20. Reviewed pest control report from 9/9/2026. 2. Photographs: Taken for documentation purposes. 3. Supervisor Notified: S. Lew 4. Notification: The person in charge during inspection, Carlos and Kelli, has been informed that the facility must close immediately. [CA]: The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider.

*MAJOR Specialist observed an old dead rodent carcass underneath aisle shelving at aisle #20. *MINOR Specialist observed approximately 5 old rodent droppings around the same area. Pest control reports from Western Exterminator Company provided for services on 08/10/26, 08/11/26, 08/04/26, 07/31/26, 07/29/26. Pest control reports noted rodent activity on 07/29 (location not provided) and 08/04 (located at christmas storage outside of receiving area). Per store director, all exclusion measures noted on the report have been addressed already. No further evidence of rodent activity found. [CA]: The premises of each food facility must be maintained free of vermin. Ensure food and food contact surfaces are protected from possible contamination at all times. Continue to follow pest control service recommendations and monitor for vermin activity. Seal all cracks, crevices, and gaps under any exterior doors to prevent entry and harborage of vermin. [COS]: Rodent carcass and droppings were removed. Affected areas were cleaned and sanitized. Pest control company was called and have been scheduled for a service today per Store Director.
