One live cockroach observed in a sticky trap maintained on the floor in the corner near the back door. Per staff, the facility receives a monthly pest control service. [CA]: The premises of each food facility must be maintained free of vermin. Ensure that the back door is kept closed where possible to prevent the intrusion of vermin. [COS] Sticky trap with cockroach inside discarded. A follow-up inspection will be conducted to ensure that the facility is free of vermin activity.
The inspector found mice, rats, cockroaches, flies, or evidence of them (like droppings) inside the business, or conditions that could attract them. Pests can spread bacteria to food and surfaces, so this is treated as one of the more serious findings.

In the kitchen area, in various dry storage containers including for creamer and cereal, observed scoops stored in direct contact with food. [CA] Ensure scoops used for food in dry storage containers are stored with the handle facing away from the food, or in a container outside of the dry storage containers. In the back storage/kitchen area, observed a knife stored between the back edge of the prep table and the wall. [CA] Ensure knives are stored in a manner that prevents contamination (such as in a container or on a knife rack).
Equipment, utensils, thermometers, cutting boards, or single-use items were not in good condition, not the right type, or not stored properly.

In the warewash area, in the 3 compartment sink, measured quaternary ammonia sanitizer at 100 PPM. [CA] Ensure quaternary ammonia sanitizer is maintained at 200 PPM to allow for proper cleaning and sanitizing of utensils and equipment.
