The food preparation sink was completely blocked by a large table. [corrective action] Remove the table so that the sink is accessible for food preparation (such as washing vegetables).
Equipment, utensils, thermometers, cutting boards, or single-use items were not in good condition, not the right type, or not stored properly.

1. Drain pipe for the food preparation sink was missing. [corrective action] Reinstall the drain pipe and ensure waste water is directed to the floor sink underneath. 2. Observed leak from the dump sink at the bar. [corrective action] Repair the leak. [suitable alternative] Manager provided a bucket and placed it under the dump sink to catch the wastewater.

Observed flies and feathers in consumable ice. Per PIC, the ice was old from yesterday. [CA] Ensure all food, ice and drinks are free from adulteration. [COS] PIC discarded ice during time of inspection.
