Observed Vermin: - Two hatched egg sacs and one dead cockroach observed on the bench against the wall. - One cockroach egg sac observed on the floor by the pillar to the right of the bench mentioned above. - Dead cockroaches observed below the bench mentioned above. - Dead cockroaches observed below the clear acrylic placed below the storage rack at the corner by the upright freezer. 2. Photographs: Taken for documentation purposes. 3. Supervisor S. Lew was notified of the closure. 4. Notification: The person in charge during inspection, Trinh, has been informed that the facility must close immediately. [CA]: The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider.

    Nota del inspector (en inglés): Observed 4-5 bags of Japanese styled peanuts gnawed through on the metal storage rack. Gnawed through peanuts were observed on top of containers on the shelves below and on the floor underneath the storage rack. Observed opened bag of sugar with dead cockroach nymph and greyish blue cement-like material fallen from the ceiling inside the bag. Bag was maintained inside an open plastic bin with live cockroaches and rodent droppings inside. [CA] Food is adulterated if it bears or contains any poisonous or deleterious substance that may render it impure or injurious to health and shall be discarded immediately. [COS] Food items were VC&D due to contamination.

Observed numerous plastic containers of ingredients/spices that have been repackaged. [CA] All food shall be separated and protected from possible contamination. Only prepackaged nonpotentially hazardous food main be maintained in storage room.
