OBSERVED NO ADDRESS/LOCATION INFORMATION ON PREPACKED GOODS SOLD (CHEESE AND YOGURT) IN DISPLAY COOLER AT REAR OF STORE. DISTRIBUTOR NAME IS PRESENT. INSTRUCTED MANAGER TO ENSURE THAT ALL PREPACKED ITEMS ARE APPROPRIATELY LABELED WITH NAME AND PLACE OF BUSINESS OF MANUFACTURER, PACKER, OR DISTRIBUTOR.
Food came from an unapproved source, was missing required date marks or labels, or was past its use-by date. Knowing where food came from and how old it is helps prevent illness.

INSTRUCTED MANAGER TO CLEAN, SANITIZE, AND MAINTAIN AREA FREE OF MOUSE DROPPINGS. PRIORITY FOUNDATION VIOLATION, 7-38-020(a). CITATION ISSUED.
