OBSERVED PEST CONTROL LOGBOOK NOT MAINTAINED. NO UP-TO-DATE INSPECTION RECEIPTS (AT LEAST TWICE A YEAR PER CODE) OR SERVICE AGREEMENT ON SITE AT THE TIME OF INSPECTION. INSTRUCTED TO PROVIDE SERVICE AGREEMENT AND UP-TO-DATE PEST CONTROL REPORT(S), BUSINESS LICENSE, CHEMICALS USED, MAP, AND ALL NECESSARY DOCUMENTS THATS NEEDED INSIDE LOG BOOK. PRIORITY FOUNDATION VIOLATION, 7-38-020(C). CITATION ISSUED.

INSTRUCTED MANAGEMENT TO PROVIDE HOT WATER ABOVE 110F AT THREE 3 COMPARTMENT SINK AND BETWEEN 100-120F AT ALL HANDWASHING SINKS AT ALL TIMES. PRIORITY VIOLATION, 7-38-030(C). CITATION ISSUED.
