CITY OF CHICAGO FOOD SERVICE SANITATION CERTIFICATE
OBSERVED MANAGER ON SITE WITHOUT THE CITY OF CHICAGO FOOD SERVICE MANAGER CERTIFICATION. INFORMED THAT ALL MANAGERS ON DUTY SHOULD OBTAIN THE CERTIFICATE AND IT SHOULD BE ON SITE AT ALL TIMES WHILE PIC IS WORKING. PRIORITY FOUNDATION VIOLATION 7-38-012 CITATION ISSUED.
