OBSERVED BROKEN TOILET IN MIDDLE STALL OF BASEMENT GIRL'S WASHROOM AND OBSERVED NON WORKING TOILET COVERED WITH GARBAGE BAG IN RIGHT STALL IN SECOND FLOOR BOY'S WASHROOM. INSTRUCTED TO REPAIR NON WORKING TOILETS IN SAID WASHROOMS IN SAID AREAS AND MAINTAIN.(NOTE: THERE ARE OTHER PROPERLY FUNCTIONING TOILETS IN SAID WASHROOMS.)

OBSERVED NO HAND WASHING SIGN IN SECOND FLOOR GIRLS' RESTROOM. INSTRUCTED PERSON IN CHARGE TO PROVIDE AND MAINTAIN.
Staff were not washing their hands when required, or a handwashing sink was blocked, missing soap or paper towels, or used for something else. Hands are one of the most common ways germs reach food.
