OBSERVED THROUGH DISCUSSIONS WITH OPERATOR THAT ESTABLISHMENT IS USING A TIME AS A PUBLIC HEALTH CONTROL PROCESS FOR CREAMER. NO WRITTEN PROCEDURES WERE PRESENT ON SITE. AN EXAMPLE OF A TPHC FORM IS BEING SENT OUT WITH THE REPORT FOR OPERATOR TO COMPLETE AND MAINTAIN ON FILE. This violation has been called in 2022, 2023, 2024 and 2025. A citation will be issued for future non-compliance.
