Each applicant for a license to operate a public market shall file an application with the department of licenses and consumer services on forms provided by the department. The director of licenses and consumer services may require such information on the application as the director considers reasonable and necessary, including: (1) The full name, date of birth, and current residence address of the applicant. (2) If the applicant is a partnership or firm, the name, place and date of birth, residence address of all partners or persons interested therein; if a corporation, the state of incorporation, the name, place and date of birth, of all persons named in the corporation, and shall state whether or not any person named in the application has ever been convicted of any crime. (3) The proposed location of the public market and the dates on which and the hours during which the market will be operated. (4) A scaled diagram showing the site plan and layout of the market stalls with an indication of the portion thereof intended to be used in the conduct of such business pursuant to the license. (5) A list of all food market manufacturers and food market distributors at the public market. (6) The market manager's name, residence address, and telephone contact number. Inspector’s notes

The market contained 3 vendors operating without proper permitting: Sugar Butter, Forescape Farm and Picnic LLC. These vendors must discontinue operating until proper permitting is obtained. Forescape Farm may sell only their product of the farm products until a Seasonal Food Vendor permit is obtained to sell their added ingredients items. In addition the vendor list provided to the health department contains vendors that do not appear to have proper permitting or licenses: These vendors include: Twin Cities Paella, Borealis Bowls & Zambezi Kitchen. These vendors may not operate until approved permits or licenses are obtained. Of note four vendors on the vendor list provided expired licenses. Upon further investigation these vendors did have active licenses and had just given incorrect license numbers. These include: Solomons Bakery, La Loma, Yummitizers, Nautical Bowls.
