Hands clean & properly washed
Observation: Employee rang customer up at cash register; placed gloves on and began to prepare food with out washing hands between the register and placing gloves on hand. 2-301.14 When to Wash (P) CDI: Employee was notified about when to wash hands. Employee removed gloves; washed hands; placed new gloves on and went back to preparing food.
Staff were not washing their hands when required, or a handwashing sink was blocked, missing soap or paper towels, or used for something else. Hands are one of the most common ways germs reach food.

Observation: Tomatoes in prep cooler were holding at 44F due to the prep cooler lid being left open. 3-501.16(A)(2) Maintain TCS foods in cold holding at 41F or less. -P CDI: Tomatoes were removed and replaced with new tomatoes out of walk in cooler that were holding at less than 41F.
Food was not kept cold enough, hot enough, cooled fast enough, or cooked or reheated to the right temperature. Bacteria grow quickly in the "danger zone" between 41°F and 135°F, which makes this one of the most important food-safety checks.

Observation: Handsink in womens bathroom is not working. Toilet in womens bathroom is not flushing properly. Womens bathroom is closed at this time. Work order has been placed to fix issues in womens bathroom. 6-501.18 Maintain handwashing sinks; toilets and urinals clean.
