4-501.114 Maintain sanitizer at correct concentrations when being used to sanitize (P) Sanitizing solution within 3-compartment sink had a concentration of 0 PPM quaternary ammonia. Quaternary ammonia sanitizing solution shall have a concentration of 200-400 PPM or according to manufacturer's instructions. CDI: Sanitizing solution was prepared manually due to dispenser error. REHS tested sanitizing solution to be 300 PPM.

6-301.11; Priority Foundation; There was no soap available at the women restroom (employees restroom). Each handwashing sink shall be provided with a supply of hand cleaning liquid soap. CDI - soap was refilled.
