3-501.16 (A) (2) and (B) Time / Temperature Control for Safety Food; Hot and Cold Holding (P) TCS food within tall reach-in cooler near bun station were maintained above 41F. Temperature controlled for safety foods shall be maintained at 41F or less. CDI: Lettuce was voluntarily discarded. Tomatoes were sent to be cooled within walk-in cooler (prepared within 4 hours of inspection). American cheese was moved to walk-in cooler and allowed to be used within 24 hours of being placed into reach-in cooler; in compliance with facility's variance procedures.

4-601.11 (B) and (C) Equipment; Food-Contact Surfaces; Nonfood Contact Surfaces; and Utensils (C) Exhaust vent panels along burger cook line were soiled with grease accumulations. Coffee milk cooler had accumulations of food debris on the bottom shelving. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust; dirt; food residue and other debris. Clean these areas more frequently. Repeat violation; half-points deducted due to improvements from previous inspection.
