4-601.11 (B) and (C); Core; The tops of the tall reach-in refrigerators have dust accumulation. Shelving inside tall reach-in refrigerator has some dried milk residue accumulation. Metal shelving across from the warewashing area has dust and debris accumulation under the black plastic drying mats. The nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust; dirt; food residue and other debris. Increase cleaning frequency.
Dishes, equipment, or surfaces that touch food were not properly washed, rinsed, and sanitized, or sanitizer was not at the right strength. Clean surfaces help stop germs from spreading to food.

Handwashing sinks supplied & accessible
5-205.11; Priority Foundation; The handwashing sink near the ice machine was blocked by several brooms and floor sweeping dustpans. A handwashing sink shall be maintained so that it is accessible at all times for employee use. CDI- items removed.
Staff were not washing their hands when required, or a handwashing sink was blocked, missing soap or paper towels, or used for something else. Hands are one of the most common ways germs reach food.
