The physical facilities are not cleaned as often as necessary to keep them clean. The walk-in cooler floor is not cleaned regularly. Food particles and discoloration were observed between the tiles. Increase cleaning frequency and thoroughly clean and sanitize the affected areas

This poses a potential food safety concern and does not meet effective contamination prevention standards.
