The license holder did not notify customers of the presence of major allergens as an ingredient in unpackaged food items served or sold to the customer. General allergen signage needed for snacks served at bar. Keeping boxes from the snacks is sufficient to have available for consumers.

The person in charge was not ensuring that employees were effectively cleaning their hands as required. Observed hand sinks that were not properly stocked with hand soap. Employees are not washing hands when returning to work.
