The presence of insects, rodents, and other pests is not being adequately controlled or minimized. Observed moderate concentration/presence of insects (fruit flies) in restrooms/bar area. Provide necessary pest control measures until presence is mitigated. PIC indicated pest control provider would be contacted for immediate service. Facility to provide copy of service/treatment invoice upon receipt.
