Food-contact surfaces were not clean to the sight and touch. No Person in charge to discuss cleaning soiled ice machine.
Food was stored or handled in a way that could let germs or other material get into it, such as raw meat stored above ready-to-eat food or uncovered food. Cross-contamination is a common cause of foodborne illness.

5.1(O)(2) - III. Preventing Contamination by Hands - Using a handwashing sink - Operation and Maintenance
Handwashing facility is being used for purposes other than handwashing. Observed perforated sink bowl dump basket inside the hand washing sink. The hand washing sink must only be used for washing hands.
Staff were not washing their hands when required, or a handwashing sink was blocked, missing soap or paper towels, or used for something else. Hands are one of the most common ways germs reach food.

The physical facilities are not cleaned as often as necessary to keep them clean. Observed trash and debris accumulated behind the bar beneath equipment. This area must be thoroughly cleaned and maintained to ensure sanitary conditions.
Dishes, equipment, or surfaces that touch food were not properly washed, rinsed, and sanitized, or sanitizer was not at the right strength. Clean surfaces help stop germs from spreading to food.

The ice needs to be discarded and the interior properly cleaned and sanitized. 2. Observed residue buildup inside the soda gun nozzle where soda comes out of. Must be cleaned and sanitized.
