4.5(A)(1) - VII. Protection from Contamination - Equipment Food-Contact Surfaces and Utensils: Clean to Sight and Touch
Food-contact surfaces were not clean to the sight and touch. Observed biofilm growth inside of the ice machines. PIC discarded all ice and the interior surfaces were thoroughly cleaned and sanitized before returning the machine to uses.
Food was stored or handled in a way that could let germs or other material get into it, such as raw meat stored above ready-to-eat food or uncovered food. Cross-contamination is a common cause of foodborne illness.

"OBSERVATION: The drain tubes of the ice macine were inside of the floor drain. Drainage system prevention A drain originating from equipment in which food, portable equipment, or utensils are placed, shall contain an indirect connection as specified in the Ohio plumbing code. CORRECTIVE ACTION: Operator will install air gaps as required."
