3.2(Y) - Miscellaneous Sources of Contamination
Miscellaneous sources of contamination observed. Observed water dripping onto cans and covered containers inside the beer cooler at the bar. PIC was advised to monitor and address the source of the condensation to prevent recurrence.
Food was stored or handled in a way that could let germs or other material get into it, such as raw meat stored above ready-to-eat food or uncovered food. Cross-contamination is a common cause of foodborne illness.
More about food not protected from contamination6.4(B)(1) - Cleaning- Frequency and Restrictions
The physical facilities are not cleaned as often as necessary to keep them clean. 1. Observed dust accumulation on the HVAC vents above the food preparation area. Vents must be cleaned more frequently to prevent contamination. 2. Observed dust and debris accumulation under the beer coolers at the bar. Must be cleaned.
Dishes, equipment, or surfaces that touch food were not properly washed, rinsed, and sanitized, or sanitizer was not at the right strength. Clean surfaces help stop germs from spreading to food.

After being cleaned, food-contact surfaces and utensils were not sanitized using an effective or approved combination of exposure time, chemical concentration, temperature, and/or pH. Observed that chlorine sanitizer was not being dispensed into the dishwashing machine, as indicated by a 0 ppm reading on the test strip after a complete cycle. PIC identified that a line feeding the sanitizer into the machine had become disconnected. The issue was promptly corrected. After the repair, PIC verified that the chlorine sanitizer concentration was dispensing at the appropriate level using test strips.
Food was stored or handled in a way that could let germs or other material get into it, such as raw meat stored above ready-to-eat food or uncovered food. Cross-contamination is a common cause of foodborne illness.
More about food not protected from contamination4.5(D) - Nonfood-contact surfaces - cleaning frequency

More about cleaning or sanitizing problems6.4(N)(1) - Maintaining premises: unnecessary items and litter.
