Observed multiple containers of bean and cheese mixed for pupusas on a prep counter at 73 F. These have been out for 1.5 hours. Potentially hazardous foods must be held at 41 F or below or 135 F or above. Containers moved to refrigeration.

Consumer advisory on menu needs a disclosure and a reminder next to the specific menu items that require it. The reminder is provided on the bottom of the menu, but a disclosure (a clearly written statement that specific food items are raw or can be cooked to order OR an asterisk *) needs to be placed next to the food items that require it matching the reminder footnote on the bottom of menu. Food was not kept cold enough, hot enough, cooled fast enough, or cooked or reheated to the right temperature. Bacteria grow quickly in the "danger zone" between 41°F and 135°F, which makes this one of the most important food-safety checks.
