Facility was remodeled without approval in spring of 2024. Submit one set of plans, construction application, specification sheets for all new equipment, finish sample board, grease trap letter from the sanitary district and plan check fee of $1,314.00 for a minor remodel. The fee covers up to 8 hours of plan review. Additional fees of $207.00 per hour will be charged if plan review exceeds 8 hours. Submit all of the above by March 13, 2025. Construction application and plan submission check;list provided.

Food manager certificate expired. Provide valid Food Safety Manager Certificate within 30 days. Food Manager Certificate must be kept on site. Discussed with person in charge at time of inspection.
