Observed rice in rice cooker measuring at 78*F. All PHF shall be stored at 135 and above or 41*F and below. Operator discarded 1 pot of cooked rice and advised to keep pot plugged in to maintain proper hot holding temperatures.

Food was not kept cold enough, hot enough, cooled fast enough, or cooked or reheated to the right temperature. Bacteria grow quickly in the "danger zone" between 41°F and 135°F, which makes this one of the most important food-safety checks.
