Observed nozzles of soda machine dispenser soiled and accumulating with debris. Food contact surfaces shall be maintained clean to sight. Clean and sanitize soda dispenser nozzles when next serviced.

Observed cottage cheese in self-serve bar measuring at 46.5 *F. Potentially hazardous foods at salad bars may be maintained above 41 *F but not above 45 *F and disposed of at the end of the day. Ensure any PHF measured greater than 41 *F are disposed off at the end of daily service. Repair self service unit to cold hold all PHF at 41 *F or below at all times throughout unit.

Food was not kept cold enough, hot enough, cooled fast enough, or cooked or reheated to the right temperature. Bacteria grow quickly in the "danger zone" between 41°F and 135°F, which makes this one of the most important food-safety checks.
