Multiple cockroaches observed in kitchen. Facility must be kept free of vermin. Have licensed pest control treat facility. Call EHS to schedule an inspection to reopen 24 hours after pest control treatment once no activity is observed.

Observed one live cockroach below dishwasher and several dead cockroaches in tray washing room. Observed one dead cockroach adjacent to trash. Ensure licensed pest control operator services tray washing room and trash enclosure for cockroaches. Clean floors, clean and sanitize equipment contact surfaces, and keep door closed. Email pest control reports inspector.
