Have professional pest control conduct service to eliminate cockroach infestation. Seal all cracks, holes and crevices to keep cockroaches out of the facility. Contact this department 24 hours after professional pest control has conducted service for a re-inspection to verify infestation has been eliminated. Facility may not reopen if any evidence of cockroaches are observed, including but not limited to any live or dead cockroaches, or cockroach eggs.

Observed multiple cockroaches at different stages of life cycle. Observed cockroach egg by food prep sink on the floor. All activity observed in the kitchen. Pest control services under contract. Manager stated pest control conducts service monthly. / Facility must close at this time. Immediately contact pest control to service facility. Remove all cockroaches and cockroach eggs from facility. Move equipment around to effectively clean and sanitize facility. Contact this department for a re-inspection no less than 24 hours from last pest control service. Cannot re-open if evidence of cockroaches found.
