Prepackaged bread observed on sales floor observed lacking label. Per person in charge, bread is received from bakery located on Canal Street. Invoices could not be obtained at this time. Provide invoices to verify all food is obtained from an approved source. Ensure all prepackaged food bears an approved label, including, but not limited to, the following: common name of product, quantity/weight/volume, ingredients including allergens. Prepackaged bread was removed from sales floor during inspection. Discontinue selling product that does not bear an approved label.

Pepsi fridge and white merchandiser fridge both had ambient temperatures of 58F. Water was dripping from the Pepsi fridge (unclear if there is a condenser fan issue). Ensure potentially hazardous foods cold hold at 41F or below. Naked Juiced in fridge stated perishable, keep refrigerated and these items were discarded. Other items did not require refrigeration in a sealed/unopened bottle. Repair refrigerators.
