Numerous dead cockroaches observed behind refrigeration units and cooking equipment in the kitchen. Observed two adult dying cockroaches by the food prep sink and under the dishwasher. Observed multiple live cockroaches in the refrigeration gasket of the beverage refrigerator by the buffet line. Observed cockroaches at different stages of their life cycle, including one cockroach with an egg. Operator stated facility is under a pest control contract and was last at the facility in the first half of September. Facility must close due to active cockroach infestation. Facility must immediately contact pest control services to eliminate cockroach infestation. After pest control has completed service, facility must clean and sanitize floors, walls, and all food contact surfaces. All cracks, holes, and crevices must be sealed using materials that are smooth, durable and easily cleanable and non-absorbent. No spray foam is allowed. Contact this department 24 hours after the last pest control, at the earliest, to verify cockroach infestation has been eliminated. No evidence of cockroaches can be observed, including but not limited to, eggs and/or live or dead cockroaches. Submit a copy of the pest control report through email prior to calling.

Observed the following: 5 large cockroaches in the front cook line area on the floor, 2 live cockroaches climbing up the hood at the front cook line, many dead in a trap behind the ice machine, many dead inside the refrigerator next to the ice machine, one dead in the dining area near the buffet, many dead flies in the front kitchen and the walk-in refrigerator, multiple live flies around the back kitchen. Facility may not be operating with active insect infestations. Have license pest control come treat facility. Clean and sanitize all food contact surfaces. Repair all holes, cracks, and crevices in facility. Contact Environmental Health Services 24 hours after pest control treatment once no activity is seen in facility.
