[CA] The floor surfaces shall be smooth, of durable construction, and of nonabsorbent material that is easily cleanable. Make plans to replace/remodel facility with approved flooring. Observed walls in the back storage behind the three-compartment sink composed of wood. [CA] Repair the wall with a durable, smooth, nonabsorbent, and easily cleanable surface. Observed square hole in the ceiling by the roof access ladder. [CA] Repair holes in structure and maintain in good repair to prevent pest harborage. Heavy accumulation of debris observed on floors in the back storage area, especially underneath sinks and shelves. [CA] Walls and/or floors shall be kept clean. Regularly clean under equipment to prevent accumulation of debris.

Numerous packages of taquitos and hot dogs maintained inside the same refrigerator measured at 45F. Ambient temperature of the refrigerator measured at 45F. Thermostat on the outside displayed a temperature of 38F. [CA] PHFs shall be held at 41°F or below or at 135°F or above. [COS] PHF items inside the refrigerator were relocated into the walk-in refrigerator to immediately cool.
