Floor, walls, ceilings: built,maintained, clean
1. There is a gap under the middle ice cream freezer by the drain pipe. 2. There are gaps between the stainless steel corner guards by the entrance closest ot the bathroom. 3. There are gaps around the handwash sink drain pipe. [CA] Repair holes in structure and maintain in good repair to prevent pest harborage.
Dishes, equipment, or surfaces that touch food were not properly washed, rinsed, and sanitized, or sanitizer was not at the right strength. Clean surfaces help stop germs from spreading to food.

1. Observed Vermin: Documented in the following areas: - A live nymph crowling on the floor from under the ice machine. - A live adult cockroach with an ootheca (egg sac) crawling on the floor between the ice machine and the freezer. - Numerous live adult cockroaches on sticky trap under the middle freezer. - Several dead cockroaches on a sticky trap under the storage rack by the bathroom kitchen entrance. - A few dead cockroaches behind the ice machine. - Dead nymphs found in the restroom floor and outside the restroom. - Reviewed pest control reports from May and June. Cockroach activity was noted on both reports. 2. Photographs: Taken for documentation purposes. 3. Supervisor Notified: Suzanne Lew 4. Notification: The person in charge during inspection, [Name], has been informed that the facility must close immediately. [CA]: The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider.

HANDLE TO ICE SCOOP WAS TOUCHING THE ICE. [CA] Handle to ice scoop shall be stored so that it is not in direct contact with ice. Always store in a scoop rack. CARDBOARD BOXES IN USE TO LINE SHELVING INSIDE THE REFRIGERATION. [CA] Discontinue the use of cardboard boxes to line shelves/flooring, use approved equipment made of materials that is easily cleanable, durable, smooth, and nonabsorbent.

DIRTY MOP WATER WAS LEFT IN A MOP BUCKET WITH DIRTY MOP WATER. [CA] After use, dirty mops water shall be dispose off and mop shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies.
