Notification: The person in charge during inspection, Hung, has been informed that the facility must close immediately. [CA]: The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider.

Chlorine sanitizer at the dishwasher was measured at 0 PPM while actively ware washing. [CA] Ensure to maintain the chlorine concentration at 50 PPM at the dishwasher. [SA] One of the two compartments of the two compartment sink were filled with a chlorine solution at 100 PPM. Facility may use the machine to wash and rinse, but must sanitize manually at the sink until the dishwasher has been repaired.

In the kitchen area, at the only hand sink, staff tried to fix a leak at the drain pipe and left the hand sink inoperable. [CA] An operable hand wash station shall be available at all times for hand washing, food preparation and the washing of utensils and equipment. Repair within 24 hours. [SA] Staff to wash hands at the mop sink directly next to the hand sink. The mop sink faucet is at the same height as the hand sink with the paper towels and soap available as well.

Hands clean, properly washed; gloves used properly
Staff were observed manually washing dishes with a set of disposable gloves on then moved on to prepare shrimp by moving the pot of shrimp without changing gloves and washing hands. [CA] Properly wash hands with soap, warm water and dry using single use paper towels as required. Gloves shall be properly used and changed when required. [COS] Staff removed gloves and washed hands prior to changing tasks.

[COS] Staff drained the bucket of shrimp into a colander and moved to walk in cooler.
