Observed 1 live German cockroach in dining area of facility. No activity observed in kitchen. Observed last pest control service report dated 01/12/2026. Per staff, no cockroach activity has been observed. [CA] Owner eliminated cockroach during inspection. Clean and sanitize area of dead cockroaches. Continue with pest control service to monitor for cockroaches. Any evidence of live cockroach activity will result in facility being ordered closed. Per owner, they will call for pest control service twice a month.

Beef soup is at 47F inside the 2 door reach in; per PIC, food was cooked and cooled the day before. [CA] After heating or hot holding, Potentially Hazardous Foods (PHFs) shall be cooled rapidly from 135°F to 70°F within 2 hours and from 70°F to 41°F within 4 hours. Approved cooling methods include: 1) Placing the food in shallow pans 2) Separating the food into thinner or smaller portions. 3) Using rapid cooling equipment (Ex. blast chiller) 4) Using containers that facilitate heat transfer (ex. stainless steel) 5) Adding ice as an ingredient. 6) Using ice paddles 7) Using an ice bath and stirring frequently 8) Accordance with a HACCP plan. Cooling foods shall have enough space around the containers for cold air to circulate, be loosely covered, or uncovered and stirred as frequently needed to evenly cool. Several potentially hazardous foods are held above 41F inside the prep table with top well in front of the cook-line and 2 door reach in in the back of the facility. Several potentially hazardous foods are stored at room temperature. Per PIC, foods inside the prep table with top well are stored in unit every morning, less than 1 hour per PIC; ambient of unit is 61F and thermometer in exterior of unit is 52F. Per PIC, food inside the 2 door upright unit are stored in unit everyday from the freezer. Per PIC, food at room temperature have been out for about half hour. [CA] PHFs shall be held at 41°F or below or at 135°F or above. [SA] Food in prep table with top well and 2 door reach in were relocated to another unit able to cold hold PHFs at 41F or below. [COS] food items over 4 hours were voluntarily discarded (see VC&D). Service technician arrived on site to service refrigeration units.
