--48 HOUR NOTICE— It was discussed with the PIC that a change of ownership had occured less than one month prior. Current records do not indicate that the permit application and facility evaluation packet have been received by this agency, therefore, this facility is currently operating without a valid health permit. Due to operating without a valid health permit, facility will additionally incur a 25% of annual permit fee. Submit permit application and facility evaluation packet in person or via email to: DEHWEB@deh.sccgov.org Failure to comply within 48 hours will result in facility closure and additional enforcement, including but not limited to reinspection(s) and additional cost recovery fees. A facility found to be in continued operation without a permit may incur a penalty of three (3) times the operating permit fee, and fee(s) for re-inspection(s) charged at the current hourly rate approved by the Santa Clara County Board of Supervisors. Penalties and fees incurred due to enforcement activities must be paid in full prior to the issuance of an operating permit. The food worker stated the chicken measured at 60F was removed from the freezer approximately 2 hours prior. B. The food worker stated the mayonnaise was left at ambient temperature for approximately 1 day. [COS] A. The chicken was placed in the reach in freezer to cool to 41F within 2 hours. B. Observed containers of frozen meats at the three compartment sink counter thawing at ambient temperature [CA] Frozen potentially hazardous food shall only be thawed in one of the following ways: 1) under refrigeration that maintains the food temperature at 41°F or below, 2) completely submerged under potable running water for a period not to exceed two hours at a water temperature of 70°F or below, and with sufficient water velocity to agitate and flush off loose particles into the sink drain, 3) in a microwave oven if immediately followed by immediate preparation, 4) as part of a cooking process.

[CA] Handwash sink shall be fully stocked with soap and single use towels at all times. [COS] Employee restocked the soap and paper towel dispensers in both the restroom and the kitchen. **MINOR* 2) At the only handwash sink in the kitchen, the top of the soap dispenser appears to be broken off.
