OBSERVED NO CITY OF CHICAGO CERTIFIED FOOD MANAGER WITH VALID ORGINAL CERTIFICATE DURING TIME OF INSPECTION. INSTRUCTED TO PROVIDE AND MAINTAIN AS REQUIRED. PRIORITY FOUNDATION VIOLATION. #7-38-012. NO CITATION ISSUED
