6-301.12; Paper towels were not available at the handwashing sink closest to the 3-comp sink. Provide paper towels or approved alternative for hand drying at each handsink. CDI- paper towels were provided.
Staff were not washing their hands when required, or a handwashing sink was blocked, missing soap or paper towels, or used for something else. Hands are one of the most common ways germs reach food.

4-203.12; Priority Foundation; The thermometer in the prepared meals display case is not working. Ambient air and water temperature measuring devices that are scaled only in Fahrenheit shall be accurate to +/- 3F in the intended range of use. CDI- work order was placed to fix this issue. All food temperatures are at 41F or less in this case.
Equipment, utensils, thermometers, cutting boards, or single-use items were not in good condition, not the right type, or not stored properly.
