4-501.114(C) A quaternary ammonium sanitizer shall have a minimum temperature of 75F and a concentration indicated by the manufacturer's directions. (P) Use room-temperature water (75F-80F) when mixing quat sanitizer to an effective concentration range of 150-400ppm. - Observed the quat sanitizer mixing with water at 130°F; and the concentration is below 150 ppm. Corrected during inspection (CD: - the quat sanitizer was remixed to 200 ppm with water at 80°F.

Discussed the matter with the person in charge (PIC). First observation; no point at this time.

6-501.12 Cleaning; Frequency and Restrictions (C).(A) PHYSICAL FACILITIES shall be cleaned as often as necessary to keep them clean. 6-501.11 Repairing - Premises; Structures; Attachments; and Fixtures - Methods (C)Floors; walls; and ceilings including the attachments such as soap and towel dispensers; light fixtures; and heat/ac vents shall be maintained in good repair. - Observed dust accumulation on the ceiling (near the air circulation unit) of the walk-in cooler room; standing water was found on the floor on the right side of the walk-in cooler door. - Observed debris accumulation under the shelf in the walk-in cooler room. - The Walk-in cooler room: observed the floor guard with rust and deterioration; which is not easy to keep clean. Discussed the matter with PIC.
