The physical facilities are not cleaned as often as necessary to keep them clean. Observed build up of grease under the fryers. PIC stated that the floors and fryers are being replaced.
Dishes, equipment, or surfaces that touch food were not properly washed, rinsed, and sanitized, or sanitizer was not at the right strength. Clean surfaces help stop germs from spreading to food.
