Observed one live cockroach on sticky trap below 3-compartment sink. Facility shall be kept free of vermin at all times. Hire licensed pest control operator to service facility for cockroaches. Remove cockroach on sticky trap from facility. Clean and sanitize area. Monitor for activity. Seal holes/cracks and crevices in facility to exclude vermin and keep doors closed. Remove raid from facility. Bring copy of most recent pest control report to re-inspection for inspector to review.
