No cold water dispensing out of hand wash/food prep sink. Ensure sink dispenses both hot and cold water to ensure proper hand washing is done as necessary. Immediately repair to provide hot and cold water at this sink.

Staff were not washing their hands when required, or a handwashing sink was blocked, missing soap or paper towels, or used for something else. Hands are one of the most common ways germs reach food.
