Employee observed washing hands in restroom facility and proceeded to don disposable gloves in food preparation area. Hands shall be washed properly at approved hand wash sink prior to donning gloves. Restroom hand sink is not approved. Employee was stopped and instructed to use designated hand sink in food preparation area.

Observed broken/missing tiles under the dishwasher, side cabinets, and under the back refrigeration units. Wall panels are broken/missing behind the dishwasher. Floors and walls must be build and maintained. Repair the floors and walls in those areas. Ensure caulking is not damaged around the kitchen.
