Liquid soap was not available in the employee restroom. [Corrective action] Provide soap in dispenser at all times to ensure the employee properly wash their hands before returning to any food handling activities.

Staff were not washing their hands when required, or a handwashing sink was blocked, missing soap or paper towels, or used for something else. Hands are one of the most common ways germs reach food.
