Facility lacks thermometer to measure PHF items. [CA] An accurate easily readable metal probe thermometer (accurate to +/- 2°F) that is designed to measure the temperature of thin masses shall be provided and readily available to accurately measure the temperatures of potentially hazardous foods.
Equipment, utensils, thermometers, cutting boards, or single-use items were not in good condition, not the right type, or not stored properly.

Mop placed inside mop bucket. Facility does not have any mop racks or devices available to store mop. [CA] After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies.

Layout of facility does not correspond to previously approved plans. - Ventilation hood and cook line has been relocated. - Water heater has been relocated from the floor level to upstairs storage. - Water heater on approved plans was gas powered. Current water heater is electric and undersized. - Mop sink has been relocated. - Hand wash sink has been relocated. At time of inspection, facility lacked any time of sanitizer. [CA] Manual sanitization shall be accomplished in the final sanitizing rinse by contacting a chlorine solution of 100 PPM concentration for at least 30 seconds or by contacting a quaternary ammonium solution of 200 PPM concentration for at least 60 seconds. [COS] Owner obtained chlorine bleach. * Note: Manual warewashing shall be accomplished by using a three-compartment sink where the utensils are first precleaned, washed with hot soapy water, rinsed with hot water, sanitized with an approved method, and air dried. Use 1 TBS of bleach (5.25%) per 1 gal. of water for chlorine solution (100 PPM). Use 1 tbs of quaternary ammonium (10%) per 2 gal. of water for quaternary ammonium (200 PPM).
