Observed one live nymph cockroach on the floor under the ventilation hood. [CA] Keep facility clear of vermin. Facility was kept closed. See notes below.

Equipment, utensils: Approved, in good repair, adequate capacity
1) Observed the walk-in freezer was leaking fluid from a pipe and into a bucket on the floor. 2) In the truck wash room: observed the 3-compartment sink's pipe was leaking water onto the floor. The faucet was also loose. Operator stated a plumber was going to come today. [CA] Repair freezer, pipes, and faucet.
Equipment, utensils, thermometers, cutting boards, or single-use items were not in good condition, not the right type, or not stored properly.

In the room with kitchens 4 and 5: observed the middle handsink was missing its drain pipe and was draining directly onto the floor. Another handsink was located nearby. [CA] Repair plumbing for handsink so that the handsink is operable.
Staff were not washing their hands when required, or a handwashing sink was blocked, missing soap or paper towels, or used for something else. Hands are one of the most common ways germs reach food.

In the restroom: observed the toilet paper roll was stored on top of the dispenser. [CA] Store toilet paper roll in an approved dispenser.

1. Observed Vermin: Documented in the following areas: In the room with kitchens #4 and 5: on a large sticky trap on the floor under a storage shelf near the rear door/exit: observed several dozen cockroaches, of which approximately 5 were alive. No other evidence of cockroaches was observed in the 334 Commercial st. facility. 2. Photographs: Taken for documentation purposes. 3. Supervisor Notified: A. Sircar 4. Notification: The person in charge during inspection, Vandana, has been informed that the facility (Kitchens 4 and 5) must close immediately. [CA]: The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. The facility (Kitchens 4 and 5) is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider.
