Rodent droppings observed in the following areas: -1 on floor near chest freezer -2 underneath prep top table near prep line cooler -20+ within the unused display case unit -2 on floor underneath cashier -1 underneath steam table -10+ within dry storage room urine stains observed on cove base within dry storage room. Two live cockroaches observed within the dry storage room (one on floor and one on ceiling panel). Photographs taken. Person in charge, Sandip, notified of closure. Supervising Environmental Health Specialist Jessica Z., notified of closure. [CA]: A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider.

Floor, walls, ceilings: built,maintained, clean
Accumulation of black grime, food debris and dirt on floors throughout the facility, warewash area. Observed accumulation of grime and grease on walls and floor sink in warewash room. [CA] Walls and/or floors in food preparation area shall be kept clean. Clean the floor sink. Observed holes in ceiling in prep and lobby area, observed a big hole in hallway leading to the back of the facility. [CA] Repair holes in structure and maintain in good repair to prevent pest harborage.

Observed new equipment in the facility for boba and indian tea. Per employee and owner (on call), the equipment is not used yet and will be used next month. Prep area seems to be extending from the past inspection visits. The area is extending into what was originally customer lobby. The floors and ceilings are not approved for food prep. The facility was originally approved [CA] Cease and desist all operations, until final plan check approval has been received from this department. A person proposing to build or remodel a food facility shall submit plans and specifications for review and shall receive plan approval prior to starting any new construction or remodeling.
