1. In the back of house area, there are several openings into the ceiling from either pipes or moved/misplaced ceiling tiles. 2. In the back of house area, at the turbo chef oven area, the wall has an accumulation of dust and grease. 3. In the back of house area, when the hand sink was turned on, an offensive odor was produced from the direction of the floor sink/waste pipe. [CA] Ensure to maintain the floors, walls, and ceilings of the facility clean and in good repair.
