Observed raw chorizo and salmon on top of ready-to-eat meal packages. [CA] Bulk milk container dispensing tubes shall be cut on the diagonal leaving no more than one inch protruding from the chilled dispensing head.

Observed 2 display coolers, an oven, soft serve ice cream machine, and a blast chiller that were not functioning and no longer in use. At time of inspection, a plastic container was used to catch oil/grease from hood above cooking equipment. [CA] Surfaces of equipment shall be smooth, easily cleanable, nonabsorbent, durable, and maintained in good repair.
Equipment, utensils, thermometers, cutting boards, or single-use items were not in good condition, not the right type, or not stored properly.

At time of inspection, the following food items in the 2-door reach-in cooler were above cold holding temperature: 1. Aoli at 47 degrees Fahrenheit 2. Ceasar dressing at 47 degrees Fahrenheit 3. Vegetable base at 47 degrees Fahrenheit. Per food employee, some food items were prepared 4 days prior and others in the morning, more than 4 hours before time of inspection. The following food items in the open air display cooler were above required cold holding temperatures: 1. Macaroni salad at 49 degrees Fahrenheit. 2. Tuna salad at 46 degrees Fahrenheit. 3. Chicken RTE meal at 51 degrees Fahrenheit. Per food employee, food items were prepared the day before. [CA] PHFs shall be held at 41°F or below or at 135°F or above. [COS] PHF items in the 2 door reach cooler and open air display cooler were VC&D.
Food was not kept cold enough, hot enough, cooled fast enough, or cooked or reheated to the right temperature. Bacteria grow quickly in the "danger zone" between 41°F and 135°F, which makes this one of the most important food-safety checks.

****MAJOR****At time of inspection, handwash sink by sushi prep did not have soap or paper towels. Handwash sink by 3-compartment sink did not have soap and access was blocked by two trash cans. [CA] Provide handwashing cleanser in dispenser at handwash stations at all times. Single-use sanitary towels shall be provided in dispensers. [CA] Handwashing facility shall be clean, unobstructed, and accessible at all times. [COS] Handwash sinks were stocked with soap and paper towels. Trash cans were removed from handwash sink by 3-compartment sink. Handwash sink by deli/sushi area entrance is not functioning and blocked by equipment. [SA] Food employees will use the handwash sink in the sushi prep area and by the 3-compartment sink until broken hadwash sink is corrected.
Staff were not washing their hands when required, or a handwashing sink was blocked, missing soap or paper towels, or used for something else. Hands are one of the most common ways germs reach food.

Observed meat slicer with meat food residue on blade and base. Per food employee, meat slicer was last used by morning shift more than 4 hours before time of inspection. [CA] In use preparation surfaces shall be cleaned at least every 4 hours or as required. [COS] Food employee cleaned meat slicer. Observed washed dishes being sanitized in a quaternary ammonium solution in 3-compartment sink that measured 0 ppm. [CA] Manual sanitization shall be accomplished in the final sanitizing rinse by contacting a chlorine solution of 100 PPM concentration for at least 30 seconds or by contacting a quaternary ammonium solution of 200 PPM concentration for at least 60 seconds. [COS] Food employee remade sanitizer solution.
Dishes, equipment, or surfaces that touch food were not properly washed, rinsed, and sanitized, or sanitizer was not at the right strength. Clean surfaces help stop germs from spreading to food.

Sliced turkey measured at 48 F stored above the fill line within the sandwich preparation refrigerator. [CA] PHFs shall be held at 41°F or below or at 135°F or above. Discontinue the practice of filling product above the preparation refrigerator fill line. [COS] Turkey above the fill line relocated to be cooled to and held at 41F or below.
