FOUND NO EMPLOYEE HEALTH POLICY ON SITE. INSTRUCTED MANAGER TO PROVIDE A VERIFIABLE ACKNOWLEDGEMENT OF THEIR RESPONSIBILITY TO REPORT INFORMATION ABOUT THEIR HEALTH AND ACTIVITIES AS THEY RELATE TO DISEASES THAT ARE TRANSMISSIBLE THROUGH FOOD TO THE PIC. PRIORITY FOUNDATION VIOLATION.
A required permit, food safety certificate, trained manager, or posted sign was missing or not current. These show the business is following the rules and has trained staff.

FOUND APPROXIMATELY 200- 300 DEAD SMALL FLIES THROUGHOUT KITCHEN UNDER AND BEHIND ALL COOKING EQUIPMENT AND DRY STORAGE EQUIPMENT. ALSO OBSERVED ON SHELF WHERE PANS ARE STORED. INSTRUCTED TO CLEAN SANITIZE AND MAINTAIN.
